Step 2: Membership Waiver & E-Signature


1. Membership and Fees
  • Membership is non-transferable and valid only for the individual listed in this form.
  • All membership fees are non-refundable and non-transferable, unless otherwise stated in writing by Boxing Academy LK.
  • Memberships cannot be paused or extended. Exceptions, if any, are at the sole discretion of Boxing Academy LK and are not guaranteed.
  • Promotional offers are valid only during the promotional period and are non-extendable.

2. Health & Medical Disclaimer
  • I confirm that I am physically fit to participate in physical training. I have no medical condition that would prevent safe participation.
  • I will immediately notify my coach of any injuries, pain, or medical issues I experience during training.
  • A medical certificate may be required upon request for members with known medical conditions.

3. Liability Waiver
  • I understand that boxing and martial arts carry a risk of injury. I voluntarily accept all risks involved in participation.
  • I release Boxing Academy LK, its coaches, staff, and affiliates from any liability for injuries, loss, or damage incurred during training or within gym premises.

4. Discipline & Conduct
  • I will maintain respect, discipline, and integrity during training.
  • Any form of violence, harassment, bullying, or misconduct will result in suspension or termination of membership without refund.
  • Members are expected to be punctual, attentive, and uphold the gym’s values at all times.

5. Use of Images and Videos
  • I grant permission to Boxing Academy LK to use photographs and videos of me for promotional purposes, including social media.

6. Facilities & Property
  • Members must treat equipment and premises with care.
  • Damage caused by negligence may be subject to repair or replacement charges.

7. Termination of Membership
  • The Academy reserves the right to suspend or terminate any membership due to breach of terms, safety risks, or inappropriate conduct.


Step 3: PAR-Q Questionnaire

If you’re aged 15–69, the PAR-Q will tell you if you should check with your doctor before significantly changing your physical activity patterns. If you’re over 69 years and aren’t used to being very active, check with your doctor. Please read each question carefully and answer honestly by ticking YES/NO.

Has your doctor ever said you have a heart condition and that you should only do physical activity recommended by a doctor?
Do you feel pain in your chest when you do physical activity? In the past month, have you had chest pain at rest?
Do you lose balance because of dizziness or do you ever lose consciousness?
Do you have a bone or joint problem (e.g. back, knee or hip) that could be made worse by physical activity?
Is your doctor currently prescribing medication for your blood pressure or heart condition?
Do you know of any other reason why you should not take part in physical activity?

If you answered YES to one or more questions:
You should consult with your doctor to clarify that it’s safe for you to become physically active at the current time.

If you answered NO to ALL of the questions:
It is reasonably safe for you to participate in physical activity, gradually building up from your current ability level.
